Two Months before

Start sorting and get rid of not needed items
Research moving companies
Create a moving binder to include: moving quotes, records (medical, dental, financial, pets, education), moving packing list, moving checklist
Start researching your new community
Ask doctors for referrals in your new place

Order supplies: moving boxes, tape, bubble wrap, permanent markers, dish barrel boxes, wardrobe boxes
Do Not Ship List. Identify all items you do not need or are not allowed to ship and sell them online (e.g ebay or craigslist) or donate them (keep the receipt for tax purposes)
Make an inventory list
Take photos or videos of high-price items. Note any existing damage to your furniture. This will come in handy if you run into issues with the moving company or insurance.
Contact health clubs and organizations to cancel memberships
Plan a garage sale
Send an email to all friends and family notifying them of the move and your new address and contact details. Ask them for any tips.

Choose your moving company and confirm
Setup a box-labeling system: number boxes, color coded by room and use a master packing list
Start packing room by room. Start with the rooms least used.
Label boxes clearly
Separate valuables. Those should probably come with you and not in the moving van.
Address Change. In the US, file with USPS.gov
Notify important parties: bank, finance, employer HR, magazines, newspapers, credit cards, insurance, home care service providers
Notify utilities: telephone, cell phone, cable/satellite/ ISP, sewer, electric, water, gas, trash collection, Netflix
Forward your medical records
Start using items you will not be moving such as frozen food and cleaning supplies
Arrange for utilities at your new address
Buy suitcases for items you will take in the car with you

Notify employer of your moving date and plan days off around it
Arrange for a baby and/or pet sitter for the move day.
Confirm with the moving company
Make travel arrangements for your pets
Service your car. You definitely don't want the movers waiting for you while you take care of a leaking engine.
Notify of your move: accountant, attorney, doctor, dentist, financial advisor, health insurance provider and agent, school
Notify government offices: city, state vehicle registration, social security administration, State and Federal Tax Bureaus (IRS)
Mark boxes you will need as soon as you arrive as "Load Last"

If moving, defrost freezer at least 24 hours before the moving day
Confirm moving time, date, addresses and mobile phone with the moving company
Arrange the payment to moving company. Ensure you have enough cash for tips to the movers and for the drive and any unexpected expenses.

Place protectors (door, floor)
Confirm the truck is from YOUR moving company. Its USDOT number should match the order confirmation
Ensure "Load Last" boxes are loaded last
Double check the inventory list made by the movers. Only sign it afterwards.
Take meter readings (gas, water, electricity) by writing them in your moving file as well as a picture using your phone.
Ensure nothing is left in the house and that you have all documents, money and valuables with you.
Leave a note with your Forwarding Address to future residents so that they can contact you if needed. Include a few stamped & addressed envelopes which they could easily use.
